Rates

$130.00 per 45-55 minute session. This includes weekly single submission insurance paperwork. Any other paperwork/billing work and emails of two or more paragraphs will cost an additional $25.00 for time and processing. Payment is expected at the beginning of each session. We will send a payment request from PayPal for any overdue fees. 

Insurance

Dawna is in-network with Aetna and Humana, she accepts some insurance plans as an out-of-network provider. Your health insurance or employee benefit plan may cover services in whole or part. Please check your coverage carefully by asking the following questions:

Do I have mental health benefits?

What is my deductible, and has it been met?

How many sessions per calendar year does my plan cover?

How much does my plan cover for an out-of-network provider?

What is the coverage amount per therapy session?

Is approval required from my primary care physician?

Payment

Cash, check, PayPal, and all major credit cards are accepted for payment.

Cancellation Policy

If you do not show up for your scheduled appointment and you have not notified us at least 48 hours in advance, a $55.00 cancellation fee will be charged. An email or text reminder will be sent two to three days in advance.

A typical counseling session at Zest of Life, LLC. lasts for 45-55 minutes, although some sessions may need to be longer or shorter depending on your needs and preferences. Double-length sessions can be scheduled.
